Job overview
We are looking for an enthusiastic and highly motivated Nurses with experience in Cardiology and Adult Congenital Heart Disease. You will work in a highly skilled busy CNS team caring for a diverse group of patients.

The GUCH team care for in the region of 10,000 patients with adult congenital heart disease. As an NHSE designated Level One ACHD centre we work in a network with Great Ormond Street Hospital as our referring paediatric centre. We also work across an Adult network which includes outreach services at UCLH, Luton and Dunstable and Basildon.

There are a number of sub-speciality areas that sit under the ACHD service. We have clinics in the specialist areas of Maternal Cardiology, Electrophysiology, Heart Failure (including close working with Transplant Centres). Pulmonary Hypertension and Transition.

Main duties of the job
You will work providing expert clinical nursing care to patients within a busy and dynamic service. The role is muti-facetated and includes responsibilities in providing access to patients via a helpline, supporting in-patient care and assistance in the patient pathways of interventional undergoing interventional and surgical procedures.

Detailed job description and main responsibilities
To participate in the clinical care of patients with Adult Congenital Heart Disease CHD (GUCH) and to assess the need for cardiological/medical review.
To provide expert nursing advice and support to ACHD patients and their families at all stages of their disease/illness.
To play a vital and important role in the outpatient care of patients and their families/carers.
To collaborate with the multi-professional team to ensure that the care delivered to patients and their families is quality driven and that Trust standards are set to reflect appropriate local and national standards.
To promote a patient focused approach to care in collaboration with carers and relevant health care professionals
To assist in establishing and monitoring protocols/care pathways related to Congenital Conditions.
To ensure patient confidentiality and to act as a patient advocate, contributing on relevant nursing issues.
Where appropriate, to establish links with community/charitable services to provide support for families/carers.
To work with the departmental team to identify areas where change is required or opportunities exist for improving the treatment and care of GUCH patients and work with them to initiate changes and facilitate/evaluate their implementations
To develop guidelines to support the delivery of care for GUCH patients
